In the above please give details of any sporting clubs you have been a member of, or coaching you have received, and evidence of higher level coaching during the last 2 years. Please include dates.

In the above please indicate whether you have suffered any injuries / illness that has kept you from training and competing during the last 2 years. If so, please include details.

In the above and in no more than 500 words, please provide any other details you believe support your application for a Sports Scholarship at Aberystwyth. Please also include details of your current training programme.

References and Supporting Evidence

Please supply a letter of support from a referee. That person should be someone who can comment on your sporting achievements, and who can confirm that your sporting goals are realistic and achievable, e.g. a coach or instructior / National Governing Body. You may also wish to send other supporting evidence to support your application.
